How to vote
Votes will be cast on the day at the event, which is being held at the Ratcliffe Bars following Sunday's game against Bradford City.
It'll cost £1 to vote with all monies raised being donated to Bloodwise, the chosen charity of Joe Thompson.
There will be five buckets displayed in front of the stage (one for each goal). Please collect a brown envelope, place your money inside, write your name on front of the envelope, and place the envelope into the relevant bucket.
The goal with the most envelopes will be the winner. In addition, a draw will be made from the envelopes, and the person whose envelope is drawn out will be invited on stage to present the award.
